We have staff working from Bristol Royal Infirmary and Southmead Hospital as well as in City Hall where we support Adult Social Care with initial enquiries from the public. Our team only takes referrals from hospital and social care professionals, supporting adults to return home after a stay in hospital by providing advice, signposting, and access to a wide range of practical and social support. When supporting Adult Social Care, we connect people to help from the voluntary and community sector. We know the voluntary and community sector well and can ensure that people get the support they need.
