Age UK Dudley will then check the information and send the completed form back to Centro - WMPTA, where it will be processed. New passes will be sent directly to people`s homes within four weeks of applying.

We are open 9am to 7pm Monday to Friday, 9am to 5pm Saturday, 11am to 4pm Sunday.
There is a £1.00 charge to cover the cost of administration.
- one recent passport photo
- proof of age (for e.g. retirement pension book, driving licence, passport or birth certificate, if the name on your birth certificate is different please also provide proof that your name as changed, such as a marriage certificate)
- proof that you live in the West Midlands (for e.g. retirement pension book, driving licence, council or housing association rent book, council tax bill or gas/electricity bill from the last 12 months)
