Delivered by Age UK Lambeth in partnership with Lambeth Together Neighbourhood and Wellbeing Delivery Alliance and South East London Integrated Care Board, the Ageing Well Link Worker Service has been developed to support older residents living with frailty to access the right support at the right time, helping them remain independent, connected and well within their communities.
What does this service do?
The Ageing Well Link Worker Service provides targeted, short-term social prescribing interventions for older people living with moderate to severe frailty.
The type of support provided will be focused on the following key areas as this ensures the support is focussed supporting someone living with severe frailty.
Community transport
Finding an exercise group
Finding a social group
Care and support needs
Help around the home
Minor adaptations (equipment) for daily living
Money worries
Income maximisation
Who is this service for?
To access the service, individuals must:
Be a resident of Lambeth,
Be aged 65 years or over,
Be housebound (due to physical health (frailty) rather than mental health.
