You will work with volunteers, health and social care professionals and wider voluntary community sector partners to support the implementation of the plan. In this role you will have the opportunity to work across our offices and several GP surgeries in north Southwark to continue to promote the work of SAIL and support patients referred into the service. 
If you enjoy supporting vulnerable people to live fulfilled lives, enjoy working in a dynamic and diverse team, and are good at building relationships with other professionals- we want to hear from you!
