How we can help you

Age UK Mid Mersey is proud to work alongside the VCA as part of the Healthy and Home programme, supporting people in St Helens who are being discharged from hospital.

Healthy and Home helps connect people with voluntary sector organisations and community support that can assist with social needs, improve wellbeing, and help individuals remain safe, independent, and happy in their own homes.

We will assess your needs to see what we can do to best support you or connect you with the most appropriate services available within the local community.
