Welcome home service

The Welcome home service supports clients being discharged from the West Suffolk Hospital.

We offer a variety of practical services such as shopping, cleaning, laundry, and we will also help people to access help from other appropriate Age UK Suffolk services, or external agencies where required.

 

                                                             

Visiting a client

 

The aim is to provide a holistic approach to clients to help them regain their confidence and former independence after a stay in hospital or after a visit to A&E.

 

For clients who are very vulnerable more intensive support can be offered.

 

We also provide a service to older people in Accident and Emergency, or who require discharge from the Emergency Assessment Unit, who would otherwise be admitted due to their social circumstances.

 

 

The service is free and is initially for 2 weeks but can be extended up to six weeks dependent on the clients needs.

 

The service operates from 10am - 6pm at weekends and 9am -9pm Monday - Friday
