Are you looking for a rewarding job in Health and Social Care supporting individuals to make informed choices about their health and wellbeing? An exciting opportunity has arisen to be part of the Watergate Primary Care Network within Newquay and the surrounding areas.
We are seeking someone with experience of working positively with people with health and social care needs, with an understanding of the voluntary sector in Cornwall. We welcome applications from people who are a good listener with great interpersonal skills who can empathise and provide support in a way that inspires trust and confidence. You will be confident, well organised with the ability to work in a pressurised environment, and computer literate with good IT skills. You will have time for people and be committed to supporting local communities to care for each other.
Working closely with the Watergate Frailty Team and Newquay Surgeries, this role will involve coordinating support for older people by liaising with care teams, local services, and families to identify services and resources. This may include carrying out home visits and assessments, therefore a full driving licence and access to a car is essential. The aim is to develop practical, person-centred support that meet the needs of patients and their families, helping to prevent unnecessary hospital admissions.

Reports to: Operational Director Age UK CIOS in partnership with the Watergate Social Prescribing Team Lead
Place of Work: Based in Govenek Hub, Newquay Surgeries of Narrowcliff, Newquay Health Centre and the Wellbeing Hub at Newquay Orchard
Due to the nature of the work, travel is an essential part of the role therefore a driving license and access to a car is essential. You will need to be able to travel independently to and from people’s homes as and when required.
Salary: £28,870 per annum pro-rata
Hours of Work: Full-time and Part-time available, therefore please let us know how many hours you would like.
Duration: Fixed term contract until March 2028
DBS: This role is subject to an Enhanced DBS check
