This is an exciting opportunity to work across the Charitable and Voluntary Sector, making a difference to people’s lives in North and East Cornwall. We are seeking someone with a background in Health & Social care and an understanding of local support organisations and internal services, to access needs and find opportunities for some of the most vulnerable in our community. This role would suit someone with excellent communication and interpersonal skills, able to form positive working relationships with a wide range of people. Organised and accurate coupled with the IT skills to confidently access client records on to digital databases and systems, such as CharityLog and Strata. Access to a car is essential in meeting our aspirations in supporting clients in North & East Cornwall.

Outcomes
Working within the hubs and across the community of North & East Cornwall, this role will enhance the lives of older people by offering a high-quality service within their homes, hubs, and hospital. You will be responsible for the delivery of practical support and pro-active co-ordination in accordance with an individual’s wishes and circumstance. You will offer a user-focused service, ensuring good practice, promoting independence, confidentiality, and integrity at all times. You will also complete holistic assessments within community hospitals and a persons’ home and provide support to those who attend groups for social activities and support. This may include our Hubs.

Background
Since 2022, Age UK Cornwall & The Isles of Scilly, Partners (including Chaos Group, Volunteer Cornwall, Cornwall Neighbourhoods for Change, and others) and Integrated Care Board have been working together to improve access to services, with the aim of reducing demand on acute services. Our partnership supports the community, coordinating holistic solutions to local challenges. This partnership and collaboration ensure that we have greater engagement and reach within local communities, helping the right people at the right time and developing innovative results. Hours of Work: 15 hours per week over 2 days.
Salary: £10,318.78 per annum (£25,453 FTE)
Place of Work: Hybrid role, peripatetic, working from home, based in St Austell Community Hub (PL25 5AE) and within the community (North and East Cornwall), offering outreach assessments within client’s homes.
Due to the nature of the work, travel is an essential part of the role therefore a driving license and access to a car is essential. You will need to be able to travel independently to and from client’s homes as and when required, meeting our aspirations in supporting clients in North and East Cornwall.
Duration: Fixed term until 31st March 2026
DBS: This role is subject to an Enhanced DBS check with Adult Barred list check.
