As part of our Independent Living Support Service, we offer dementia support in the home for clients living with dementia at any stage.

Our Dementia trained and dedicated support workers will work with clients and their families to put together a personalised programme of activities to provide stimulation and support to aid memory and improve well-being. This can include activities in the home as well as taking clients out into the community and also provides carers with some respite.

All our staff are fully trained and rigorously vetted in a process that includes checks with the Disclosure and Barring Service.

When is Dementia Outreach at Home offered.

We currently offer Dementia Outreach at Home from Monday to Friday. We can offer as much or as little support as you require (with a minimum of two hours a week).

How much does it cost?

Our Dementia Outreach at Home Service costs £22.50 per hour. The minimum amount of support we offer is two hours per week. Clients and their families will be invoiced at the end of each month.

How do I access this service?

If you, a relative or someone you know is interested in accessing this service, please contact us on either telephone 01732 454108 or email ils@ageuksevenoaksandtonbridge.org.uk and register your interest.

Once we have received the referral, a member of staff will get in contact to arrange a time to come and visit so we can have an informal chat about how we can support you and your family member and to complete an assessment.
