Hospital Project

 The Age UK Isle of Wight Hospital Project works within St Mary's Hospital to provide support to older Island residents.

 This support includes:

  • Activities at the Community Unit and the Gouldings Day Hub - Helping our residents to recover by encouraging daily activity.

  • Care Navigation on the Hospital Wards - Supporting residents by having the time to sit and talk to them about what they may need to get home safely. Supporting them navigate the system to get this. Provide welfare support once home.

 

  • Crisis & Community Support -  Helping those who may need some additional support to be discharged, followed up by further welfare calls and community support to avoid re-admission.
