Community Navigators - Swale

Community minded people needed for rewarding roles in the Sittingbourne and Sheppey areas, as part of a Swale Clinical Commissioning Group joint-funded project between Age UK Faversham & Sittingbourne and Age UK Sheppey.

  • Temporary contracts, funded initially until 31st March 2019.
  • Full and part-time roles available.
  • £18,500 - £21,500 pro rata, according to qualifications and experience
  • Roles available immediately.
  • The service will be jointly commissioned by Swale CCG and Kent County Council from 1st April 2019. There is a possibility that these roles may carry forward into the new contract.

We are seeking good listeners who love being involved in their local community to make a real difference to the lives of older and vulnerable people across Swale.

The Community Navigator will use a holistic, person-centred approach to improve or maintain the health, wellbeing and independence of older and vulnerable adults with complex needs.They will enable people and their families and carers to make best and timely use of the full range of available support and help across public, voluntary and community services. 

They will address social, economic and environmental issues, as well as health and care needs.   They will work over a limited time period to enable people to achieve their personal goals, address immediate concerns, and ensure ongoing support is in place.

The Navigator will be a key member of an assigned Local Care Team (multi-disciplinary teams centred around primary care), contributing to shared care planning.  They will work particularly closely with Local Team members, as well as Rapid Response and Discharge Teams, and provide a central contact point for non- clinical referrals and planning.  They will be allocated to work with a particular Local Care Team but will need to support other teams when required.

The role involves visiting people in their own homes and attending various meetings. As such, you will need to have a full driving licence and the use of a car, for which mileage expenses will be paid.

If you have a practical understanding of the challenges and barriers facing older and vulnerable people, their carers and families, and a knowledge of the role of statutory and voluntary sector agencies, you could be the person we are looking for. Experience in health and/or social care would be an advantage, but not essential. The ability to deal with older and vulnerable people in an empathetic way is a must.

Training and support will be provided to enable the post holder to fulfill their role. Because of the nature of the position, an enhanced DBS will be required.

Age U Can Domestic & Companionship Service is now recruiting…

Due to expanding our services we are looking for friendly & reliable staff to work as part of our team in delivering domestic/shopping services to our clients in their own homes.

You will need to be a driver with a car as we cover Faversham, Sittingbourne and surrounding areas.

Age U can Domestics operates five days a week (normally 8am – 6pm) and individual hours will be arranged.
