Age UK's Personalised Independence Programme (PIP) helps people over 55 to Live Well - working in partnership with NHS Kent and Medway.

Who is the Programme for?

The programme is for people over the age of 55 who need support to:

• Get information about services
• Improve confidence, independence and safety
• Improve general health and wellbeing
• Reduce social isolation and loneliness

How will this help me?

The support is entirely tailored to your needs. A Personal Independence Coordinator (PIC) will visit you at home and will listen to you about the things you may want help with. This could involve:

• Putting you in touch with various groups / services and helping you join or access them if neccessary
• Accessing equipment, adaptations and lifelines
• Providing direct support to help improve confidence and self esteem

The support for the PIC is free of charge an can last for up to 12 weeks.

Information for Health and Social Care Professionals

The Personalised Independence Programme offers Social Prescribing and direct support to clients over the age of 55.

The programme lasts for up to 12 weeks. PIP enables Health Care Professionals to refer patients to broad ranging, non-clinical support within the local community.

How can I refer?

Referral to the programme couldn't be easier. The Local Personal Independence Coordinator (PIC) will attend the Local Care Hub and can take referrals at these meetings. Alternatively, you can refer direct to the PIC via the NHS email or mobile number below.

To make the referral we will need their: name, address, phone number, date of birth and NHS number. A brief description of the identified needs or concerns would also be helpful.

Patients on the palliative care register or in nursing or residential care are excluded from the programme. We do require consent and for patients to be aware that we will be contacting.
