Hospital to Home Services | Q3 Report Oct to Dec 23

Age UK Cornwall and The Isles of Scilly is providing Hospital to Home Services in community hospitals throughout Cornwall. We are extremely proud to work alongside NHS staff, local community hubs and care providers. Together, we deliver tailored support to people and families who need our help.

The following report provides an overview of this partnership and the support provided by staff, volunteers and health professionals from October to December 2023.
