We are thrilled to share David's story from the Inclusion Matters and Community Gateway services. David's brave journey serves as a powerful testament to how early intervention can significantly impact an individual's wellbeing.
Our outreach teams skillfully evaluated a challenging situation and worked with local providers and Adult Social Care to provide better ongoing support for David.
