Regain your independence at home
A spell in hospital can leave you feeling anxious and sometimes quite vulnerable. The Out of Hospital service works with you for up to 4 weeks to reassure and help you identify what is important to you to get back on your feet and build longer-term confidence following discharge from hospital.
Who is the Out of Hospital service for?
The service is available to older people who are discharged following a stay in hospital. You must be aged 65 or over, live in Gloucestershire, be registered with a Gloucestershire GP and be happy to have a home visit from one of our team to discuss your needs and circumstances.
How can the Out of Hospital service help?
Our countywide team of paid staff and trained volunteers are available to help you maintain your independence, signpost, refer and discuss what is important for you to live confidently once you are home. Once you are home from hospital we will arrange for one of our Community Support Coordinators to visit you at home as soon as is practically possible.
Our support can include:
- A ‘Safe and Well’ Home Safety Check.
- Support to access benefits advice and guidance, e.g. Attendance Allowance. • Information, advice and sign-posting to other services relevant to your needs.
- Practical tasks such as a one-off essential food shop and collecting prescriptions, to help you settle back at home.
- A volunteer visitor for up to four weeks, providing company at home and supporting you to rebuild your confidence.
How can you be referred to the Out of Hospital service?
Any health or social care professional, relative or friend can refer you or enquire on your behalf — or you can refer yourself.
There is no charge for this service as it is funded by Gloucestershire Clinical Commissioning Group (GCCG).
Our out of Hospital Service is provided Monday to Friday 9am – 5pm
