A spell in hospital can leave you feeling anxious and sometimes quite vulnerable. Our Home from Hospital Service works with you to reassure and help you identify what is important to you to get back on your feet and build longer-term confidence following discharge from hospital.

About our Home From Hospital Service

The Home from Hospital service is available to older people who are discharged following a stay in hospital. Available to anyone over the age of 18, and be happy to have a call or home visit from one of our team to discuss your needs and circumstances.

How can we support you?

Our team of paid staff and trained volunteers are available to help you maintain your independence, signpost, refer and discuss what is important for you to live confidently once you are home.

Our support can include:
* We will meet you at home following your stay in hospital**
* We will ensure your home is warm and safe
* We will support with light household tasks to ensure you are comfortable
* We will keep in contact with you to check you have everything you need
* We will support with an initial shop for essentials food items
* We will assess your needs to see if there is any other support we can offer
* Attend healthcare appointments with you in the immediate time after your stay in hospital**
* Signpost you to our Information and Advice service if needed

**Monday to Friday within 48 hours of your discharge, time specific conditions apply

This will be an opportunity for one of our team leaders to discuss our services in more detail, to find out exactly what kind of help is needed and to plan an appropriate schedule of visits.
