Discharge Services Wellbeing Coordinator
Age UK Lincolnshire is proud to be working in collaboration with United Lincolnshire Teaching Hospitals NHS Trust and Lincolnshire Partnership NHS Foundation Trust to support residents across the county with safe, timely discharges from hospital during the winter period.
As part of this initiative, you will join a dedicated team focused on facilitating timely and smooth transitions from hospital to home, ensuring patients can recover in a safe environment where they feel most comfortable. We are looking for energetic, compassionate and proactive individuals who bring creativity, problem-solving skills, and a commitment to making a real difference. Together, we will help improve patient flow, reduce delays, and deliver tailored support that minimises the risk of readmission and empowers people to regain independence at home.
Join us as a Discharge Services Wellbeing Coordinator!
Working in partnership with Lincolnshire’s hospitals, this role helps alleviate additional winter pressures by providing timely, compassionate, and practical support to individuals recently discharged from hospital. This telephone-based position involves making wellbeing calls to those discharged without a continuing package of care, ensuring they are safely recovering at home. You will help individuals access the resources and assistance they need to recover safely and avoid unnecessary readmission.
About the Role
- A fixed-event contract until 31st March 2026
- 35 hours per week - hours will be worked between the hours of 08:30 – 19:00 to meet the needs of the business
- £23,707 per annum (FTE)
- Hybrid working
What we’re looking for:
- Experience in information and advice provision and working collaboratively within a team.
- Ability to manage a varied and demanding workload with professionalism and flexibility.
- Strong understanding of the needs of vulnerable people and excellent communication skills, both verbal and written.
- Proficiency in Microsoft Office and good local knowledge of countywide services.
- Approachable, empathetic, and proactive with strong organisational skills and a commitment to person-centred support.
- A good standard of education and willingness to undertake training.
