CompasseHub
In addition to being a resource hub for our information and services, the Compass eHub is also home to our Compass Assessment Tool.
This tool supports a simple yet ambitious goal: reducing pressure on hospitals by helping to prevent avoidable emergency admissions.
To achieve this effectively, we need to identify clients who are most at risk. Our extensive experience in delivering prevention services—and more recently, Social Prescribing—has shown us that while a client’s health is a key risk factor, it’s not the only one.
We take a 360-degree view of each individual’s situation, looking beyond health to consider the full picture of their needs and circumstances.
