Care Coordinator
Age UK Lincolnshire is an independent local charity, a brand partner of Age UK, supporting more than 6,000 people every month, making a real difference to people’s lives. We are looking for Care Coordinator based in our Park Street Office, Lincoln.

About the role
- Monday to Friday
- 35-hour contract
- Option to compress hours over 4 days
- £26,374 per annum
- Permanent contract
- Driving licence is essential
At the heart of our values-led service is the provision of safe, effective, caring, responsive and well-led person-centred care, enabling our clients to live fulfilling and independent lives with the dignity & respect they deserve at all times. We are therefore looking for someone with demonstrable knowledge, skills and experience of working with older adults; preferably in a homecare setting and who must be committed to delivering an outstanding service in accordance with the fundamental standards of the Care Quality Commission.
The role will be both rewarding and challenging and will include but is not limited to the following:
- Assisting in the selection and recruitment of Care Companions across the Lincolnshire area
- Assisting with induction and training of Care Companions including supervision and assessment of The Care Certificate (Approved Assessor Training will be provided)
- Undertake ongoing assessment, supervision and field observations of Care Companion
- Develop person-centred, outcome focused Care and Support Plans in conjunction with clients and their families accessing the service
- Provide regular care and support reviews and assessments for clients accessing the service
- Empower clients to make their own choices, and reflect this in their care plans
- Ensuring safe staffing levels; organise rotas; manage and plan care delivery to provide the highest quality of care to clients
- Respond proactively to day to day challenges and changes in care delivery requirements

Essential:
- Recent prior experience of working in a health and/or social care setting and familiar with CQC fundamental standards and key lines of enquiry
- Committed to the provision of outstanding care services for our clients.
- Will hold a full UK Driving Licence and have access to a car.
- This role is subject to Disclosure and Barring Scheme.
