Age UK Lincolnshire has an exciting opportunity for a Community Connector working in Lincoln Hospital.

About the role

  • 28 hours per week
  • Over 4 days, Monday to Friday
  • £18,965 per annum (£23,707 FTE)
  • Fixed event until 31st March 2026
  • Primary location Lincoln Hospital but the post holder will be required to travel to other locations if service demand requires

As a Community Connector you will support patients to return home after a hospital stay. To deliver the integrated model of care by supporting patients, carers and families, working closely with colleagues from across the Health and Care sector to avoid unnecessary admission to hospital and facilitate timely discharge from Lincolnshire hospitals.

Carry a caseload to deliver Holistic Care Planning, support and coordination of contact with hospital and community-based patients.  You will help work towards establishing a care package, accessing aids and equipment, signposting or referring either internally, or to partner organisations who can offer welfare benefit checks, shopping and cleaning services or social groups to help combat isolation.

Prepare grant applications to support patient discharge or prevent unnecessary hospital admission. Liaise with services and obtain quotes within a suitable timeframe, prepare application paperwork including care plans.  Oversee successful grant applications to ensure requests are completed, and make follow up calls to patients.

Participate in multi-disciplinary meetings and other hospital-based meetings and events to support both Health and Social Care staff, patients and their families.

Initiative and creative thinking is necessary to achieve the best outcome for the individual you work with and your input can dramatically improve the day to day life once the patient returns home.
