Age UK Lincolnshire is proud to be working in collaboration with United Lincolnshire Teaching Hospitals NHS Trust and Lincolnshire Partnership NHS Foundation Trust to support residents across the county with safe, timely discharges from hospital during the winter period.

As part of this initiative, you will join a dedicated team focused on facilitating timely and smooth transitions from hospital to home, ensuring patients can recover in a safe environment where they feel most comfortable. We are looking for energetic, compassionate and proactive individuals who bring creativity, problem-solving skills, and a commitment to making a real difference. Together, we will help improve patient flow, reduce delays, and deliver tailored support that minimises the risk of readmission and empowers people to regain independence at home.

As a Discharge Services Community Care Responder, you will play a vital role in delivering compassionate, person-centred care to clients in their own homes. You’ll ensure that our services are responsive and tailored to individual needs, carrying out thorough risk assessments, reviewing care plans, and providing personal care where required.

In this role, you’ll be committed to safeguarding the wellbeing of clients by responding promptly to Telecare alerts and working closely with families and external professionals. You will maintain accurate and timely documentation, foster a respectful and supportive environment, and take a proactive, flexible approach to care that adapts to the changing needs of those you support.

About the Role

  • A fixed-event contract until 31st March 2026
  • 35 hours per week
  • 8-hour shifts (1-hour break): 06:00-14:00 or 14:00-22:00
  • Service operates 7 days a week
  • Full UK driving licence and access to a car (with business insurance) is essential
  • £23,707 per annum (FTE)
  • Local offices in Boston, Grantham, Lincoln and Skegness – countywide service
  • Mileage paid at 45p a mile

What we’re looking for:

  • Excellent communication and interpersonal skills
  • Strong organisational skills and attention to detail
  • Ability to work independently and as part of a team
  • Caring, empathetic, and patient approach to client care
  • Positive attitude and commitment to confidentiality

Experience in care and an NVQ Level 2 in Health & Social Care are desirable but not essential.
