Social Prescribing is a way of linking you to sources of support within your community.

It provides you with additional help to complement your existing treatments to improve your health and wellbeing.

People who receive a “social prescription” from their GP surgery improve faster than those treated with medication alone.

What do we offer?
Our Social Prescribing Service can: 

  • Link you to local groups and activities
  • Help you make positive lifestyle changes
  • Help you to learn new things
  • Help you to make new friends
  • Refer you to other services as appropriate
  • Help make sure you are in claiming all the benefits you are entitled to
  • Ensure you have access to the information you need to make your own choices in

 

What we can’t offer

  • We can’t organise transport but we can give you information on community transport options
  • We can’t organise your medical appointments, we advise you speak to your GP
  • We are not a replacement for social care and are not designed for crisis situations


Who is the service for? The service is currently available for patients of our partner GP surgeries as listed below,

 

  • All GP surgeries in Flixton, Urmston, Davyhulme and Partington
  • All GP surgeries in the Sale area
  • The following GP surgeries in the South of the borough:

Barrington Medical Centre

Riddings Family Health Centre

Shay Lane Medical Centre (Patel)

The Family Surgery

The Village Surgery

Westwood Surgery

 


How to access the service 

You can talk to your GP or health care professional and they will make a referral to us.
You can refer yourself by calling 0161 672 9645 or email social.prescribing@ageuktrafford.org.uk
