The Home from Hospital service is for adults living in the Wokingham Borough Council area. We help people settle back home after a hospital stay. We support them for a number of weeks, with things like food shopping, light housework, collecting medication, liaising with wards and other medical professionals etc.
This service is available in Wokingham area and is also offered in Bracknell and Reading.
