Age UK East London assists people home from hospital and provides practical and emotional support in the following weeks.
What is this service?
Funded by Bart’s Health NHS Trust Whipps Cross Hospital the Hospital to Home Service is available to patients resident of Waltham Forest, Newham, Redbridge and Essex. Referrals can only be accepted through the hospital health and social care professionals. The service provides escorted discharge along with both practical and emotional support for up to 6 weeks post discharge.
