Team Leader – Social Prescribing

Job Type: Full time, permanent

Working Hours: 37.5 hours - 5 days per week, Monday to Friday.

Salary: £36,792 per annum

Benefits: 30 days annual leave, 6% Pension Contribution, Employee Assistance Programme. Hybrid Working (1-2 days working from home)

Work Location: Across GP practices, Community Venues, Age UK I Office and Hybrid remote.

We are seeking an enthusiastic and experienced team leader to support a team of social prescribing link workers, currently working across two Primary Care Networks (PCNS) in Islington

The Team leader role includes referral screening and case allocation alongside specialisms in e.g. working with specific service user groups, project development, referral pathway development, new service implementation, coaching and mentoring and multi-disciplinary service integration.

This role is well suited to an individual with experience of supporting and developing staff to achieve their goals and service objectives, whilst ensuring quality of service.  

The role requires both Team Leader responsibilities and case managing a quota of clients, triaging referrals, actively connecting people to specialist services or community services to promote independent healthy living.

Salary: £19,478 (FTE £32,464)
