Discharge Buddies Support Worker
Age UK Lincolnshire is proud to be working in collaboration with United Lincolnshire Teaching Hospitals NHS Trust and Lincolnshire Partnership NHS Foundation Trust to support residents across the county with safe, timely discharges from hospital during the winter period.
As part of this initiative, you will join a dedicated team focused on facilitating timely and smooth transitions from hospital to home, ensuring patients can recover in a safe environment where they feel most comfortable.We are looking for energetic, compassionate and proactive individuals who bring creativity, problem-solving skills, and a commitment to making a real difference. Together, we will help improve patient flow, reduce delays, and deliver tailored support that minimises the risk of readmission and empowers people to regain independence at home.
Join our team and help Age UK Lincolnshire deliver safe and effective hospital discharges, supporting the wider healthcare system. Your role will focus on enabling individuals to return home safely and remain independent, enhancing their quality of life. This service is tailored to individual needs, and you’ll contribute to delivering high-quality support while maintaining confidentiality and safeguarding standards. Please note: there are no personal care duties involved.
About the Role
- A fixed-event contract until 31st March 2026
- 30 hours per week
- Full UK driving licence and access to a car (with business insurance) is essential
- £19,047 per annum (£22,222 FTE)
- Lincolnshire – Based at Boston, Lincoln or Grantham (dependent on your local area) with the willingness to travel countywide.
- Mileage paid at 45p a mile.
What we’re looking for:
- Experience in domestic cleaning and household tasks
- Ability to work independently, follow lone working procedures, and manage time effectively
- Strong commitment to confidentiality and safeguarding
- Caring, compassionate, and reliable with a flexible, ‘can-do’ attitude
- Good communication skills (verbal and written)
- Competent in basic administrative tasks
Desirable: Basic First Aid, Manual Handling, and Food Hygiene Level 2 certification.
