Community Connector
Could you be a calm, practical presence when it matters most?
We are looking for two Community Connectors: 1 x 21 hours a week and 1 x 28 hours a week
We’re looking for someone compassionate, organised and people-focused to support older people as they prepare to return home from the Royal United Hospital in Bath.
This isn’t just a job. It’s a chance to make a real difference every day, right when people need it most.
What You’ll Do:
- 
Visit patients on the wards and chat to them about what matters most
 - 
Help connect people to local services through the Community Wellbeing Hub
 - 
Provide essential HELP packs with food and supplies for their first few days at home
 - 
Support a small team of volunteers working in the hospital atrium
