This service is free to people resident in Wokingham Borough Council, Reading Borough Council, or Bracknell Forest Council areas.

Please note, this service is not available outside Age UK Berkshire - Head Office's catchment area.

We provide a Home from Hospital service to help people in the first few days and weeks after leaving hospital. This service is FREE and offers help with cleaning and housework, food shopping, collecting medication and more. This service is offered to people living in the Wokingham, Bracknell and Reading areas.

What is the Home from Hospital service?

The Home from Hospital service provides help and support to people who are being discharged from hospital following a planned or emergency stay.

We can offer a range of help including practical support such as:
- help with shopping
- light household cleaning
- taking you to and from the GP and hospital appointments
- ordering and collecting medication
- checking in with you regularly to see how you are doing

The service lasts for a period of time after you are discharged, which can be up to several weeks.

Our experienced staff work with you to ensure that you are settled in at home after being discharged from hospital, they can also support you by providing information, advice and signposting to other services that will help you in the future.
