An innovative, free service working collaboratively with selected GP practices in Newport. We provide interventional support, linking older people with support services that can enable them to stay well and healthy in their own homes.

Our staff will visit older people at home and offer the opportunity to create a Stay Well Plan which focuses on all aspects of their life to help them maintain their health, wellbeing and independence.

If you are interested in the service ask your GP surgery if you can be referred and we will be in contact as soon as possible.
