Support Services Coordinator
Age UK Lincolnshire is expanding across our area to meet the rising demand for our fantastic services. We are looking for a Support Services Coordinator in the following area:

About the role
- Monday to Friday
- 08:30 to 16:00
- 21 hours per week
- 16,471 per annum (£27,453 FTE)
- Driving licence is essential
At the heart of our values-led service is the provision of safe, effective, caring, responsive and well-led person-centred support, enabling our clients to live fulfilling and independent lives with the dignity & respect they deserve at all times. We are therefore looking for a person with demonstrable knowledge, skills and experience of working with older adults; preferably in a homecare setting and who must be committed to delivering an outstanding service.
- The role will be both rewarding and challenging and will include but is not limited to the following:
- Develop person-centred, outcome focused Support Plans in conjunction with clients and their families accessing the service
- Provide regular support reviews and assessments for clients accessing the service
- Empower clients to make their own choices, and reflect this in their support plans
- Assisting in the selection and recruitment of Support Workers across our area of coverage
- Assisting with induction and training of Support Workers & Gardeners
- Undertake ongoing assessment, supervision and field observations of Support Workers
- Ensuring safe staffing levels; organise rotas; manage and plan service delivery to provide the highest quality of support to clients
- Respond proactively to day to day challenges and changes in support delivery requirements
Essential:
- Recent prior experience of working in a health and/or social care setting
- Committed to the provision of outstanding support services for our clients
- Will hold a full UK Driving Licence and have access to a car
- This role is subject to Disclosure and Barring Scheme
