Help Older People Flourish

Age UK Lincolnshire is a thriving local charity supporting more than 6,000 people every month across Lincolnshire. We are committed to helping older people remain independent, safe and connected within their communities.

We are currently seeking an enthusiastic, compassionate and motivated Team Leader to join our HART team.

This is a rewarding opportunity for an experienced care professional who enjoys leading and supporting others while continuing to deliver hands on care and emergency response services that make a real difference to people’s lives.

About the Role

As Team leader, you will play a vital role in ensuring the safe and effective operation of our Hospital Avoidance & Response Team. This is a dual-purpose position combining frontline support with leadership responsibilities.

You will respond to telecare activations, support people in their own homes, coordinate resources during urgent situations and lead a dedicated team to deliver high-quality, person-centred care.

  • Countywide role, based in Lincoln, Lincolnshire
  • 38.5 hours per week
  • £30,198 per annum
  • Rotational rota across 7 days
  • Shifts between 06:00 and 18:00

Your responsibilities will include:

Operational Leadership

  • Responding to telecare activations and emergency situations as a first responder.
  • Making informed operational decisions to ensure safe and effective service delivery.
  • Coordinating incoming calls and deploying appropriate resources to support individuals.
  • Providing hands-on support to people requiring care and assistance in their own homes.

Team Management

  • Leading and supporting staff during day-to-day operations.
  • Conducting supervisions, competency assessments and performance reviews.
  • Monitoring staff wellbeing, performance and compliance.
  • Supporting the recruitment, induction and development of new team members.

Training and Development

  • Ensuring mandatory training is completed and maintained across the team.
  • Assessing and supporting staff working towards the Care Certificate.
  • Mentoring and coaching colleagues to achieve their full potential.

Service Quality and Compliance

  • Completing audits and quality checks on reports and service records.
  • Ensuring compliance with CQC standards, organisational policies and best practice.
  • Accurately recording and managing operational data and documentation.
  • Promoting a culture of professionalism, respect and continuous improvement.

Rota and Resource Management

  • Assisting with rota planning and staffing allocation.
  • Managing short-notice staffing changes and ensuring service continuity.
  • Managing staffing concerns and escalating issues appropriately.

  • We are looking for an enthusiastic, compassionate and motivated individual who can effectively balance operational leadership with frontline support.

Essential

  • Strong leadership and organisational skills.
  • Understanding of the needs of older people and those at risk.
  • Knowledge of CQC requirements and quality standards.
  • Excellent communication and interpersonal skills.
  • A compassionate, patient and person-centred approach.
  • Ability to work weekends as part of a rota.
  • Full UK driving licence and access to your own vehicle.
  • Commitment to ongoing learning and professional development.

Desirable

  • Experience within health, social care or customer service environments.
  • Previous experience supervising or leading staff.
  • NVQ Level 3 in Health & Social Care.
  • Experience using electronic care systems and databases.
