This is a person-centred service that is delivered through home visits and by telephone. We work with individuals to identify their needs and service requirements, providing the necessary support and signposting to our services or other organisations that meet your needs more appropriately.
What can you expect?
Following referral to Age UK Southampton you will be visited at home at a mutually convenient date and time. The Wellbeing Team will discuss with you how things are going presently and what support you currently have, how you manage your day, what you might like to change and your personal goals for the future. The conversation is about you and is a person centred assessment. It is intended to help you to make choices that will improve your daily living, maintain you independence, and improve your general health and wellbeing.
After the visit the Wellbeing Team will record the details of your conversation. Everything that you have told us will be treated with the strictest confidence. They will then signpost you to any of the services either internally as provided by Age UK Southampton or externally that you have identified. You will be supported to access the relevant service and any information that you need will be sent to you.
If you have asked to be visited by one of our Visitor Volunteers or you would like to receive a regular call from our Telephone Support Volunteers the situation will be closely monitored. The Wellbeing Team will contact you to arrange a telephone and later home visit reviews. Any new needs will be identified.
At all times you will be treated with respect and your personal views, needs and desires will be fully taken into account.
