This is a person-centred service that is delivered through home visits and by telephone. We work one-to-one with you to identify your needs and goals, providing the necessary support and signposting to our services or other organisations that may meet your needs more appropriately.
What can you expect?
Following a referral to Age UK Southampton you will be visited at home at a mutually convenient date and time. The Wellbeing Team will discuss with you how things are going presently and what support you currently have, how you manage your day, what you might like to change and your personal goals for the future. The conversation is about you and is a person-centred assessment. It is intended to help you to make choices that will improve your daily living, maintain your independence and improve your general health and wellbeing.
After the visit, the Wellbeing Team will review the details of your conversation; they will then connect you to relevant services either internally as provided by Age UK Southampton or externally to other agencies. You will be provided with support to access the relevant service, and any information that you need will be sent to you.
If you have asked to be visited by one of our Visiting Volunteers or you would like to receive a regular call from our Telephone Support Volunteers, the Wellbeing Team will contact you to arrange a telephone review, and a follow-up home visit. Any new needs we identify will be discussed with you.
At all times you will be treated with respect and your personal views, needs and desires will be fully taken into account. Everything that you tell us will be treated with the strictest confidence in accordance with Data Protection Regulations
