Social Prescribing returns to Warwick Hospital

The Hospital Social Prescribing Team is happy to announce that they are going back into Warwick Hospital from 8th June 2021. The service is there to provide a holistic approach to health needs by linking to community support and making returning home from hospital safer and quicker. We support patients and their carers get the help they need from housing, benefits, adaptations, and helping with loneliness.
