Where is this service based?
This service is based at the Royal London Hospital in Tower Hamlets and is funded by Tower Hamlets Clinical Commissioning Group.
Who is this service for?
Aimed at residents of Tower Hamlets, this service assists vulnerable adults in getting home from hospital. Age UK East London will carry out safety checks in the home, give advice in practical and wellbeing matters, and visit regularly for a period of six weeks after discharge.
