New Service: Independence at Home

Age UK Herefordshire & Worcestershire’s new ‘Independence at Home’ service provides practical support for adults in Worcestershire to improve their independence, health and wellbeing at home, while reducing the need for health and care support.

The service which launched on December 1st offers county wide support with the following eligibility criteria:

Eligibility:

  • Adults aged 18 and over
  • Not in receipt of fully funded social care package or long-term care
  • Resident of Worcestershire (Worcestershire council taxpayer)

Short term support:

  • Providing a service to individuals to keep them in their own homes for as long as possible, in a caring and personalised manner
  • Keeping people well, at home and away from hospital
  • Reducing avoidable admissions through community support
  • Aiding discharge from hospital

The service will provide an ‘Information Line' on Saturday & Sunday 9am - 5pm, and bank holidays, for enquiries only, contact: 07503687102.

Referrals:

Following a discharge from hospital, clients can self-refer via an open referral route. To receive support to prevent an admission to hospital, a referral must be made by a health care professional.

Please note, all referrals for the Age UK H&W Home from Hospital service will now be part of the Independence at Home service.

There is no charge for this service, as it is currently jointly funded by Worcestershire County Council and Worcestershire Clinical Commissioning Groups.
