We have two Hospital Services avaliable at Age UK Lambeth, Hospital 2 Home and Welcome Home. Both offer slightly different support, please read below for more details on what they can offer you, who the service is for and how you can access the service.

Hospital 2 Home

The Hospital 2 Home team is there to assist patients have a safe and comfortable discharge from Hospital. Our worker can either help escort you home or meet you there when you arrive. This is to ensure you are comfortable and settled upon your return. They can also help with a number of other things, including:

- Collect any items you need from home in the lead up to your discharge, this can include clothes or paperwork.
- Ensure that there is enough food and other practical items in the house for your return.
- Make sure that everything is in working order like the gas and electricity.
- Assures minor adaptations and furniture is moved on the day of discharge and leaves you feeling safe and happy to be back home.
- They can also link in with other Age UK Lambeth services to prevent future hospital readmission such as a cleaner who is part of the Hospital 2 Home team.

Who is the service for?

The Hospital 2 Home team work across Kings College Hospital and St Thomas Hospital however the service is open to anyone living in Lambeth.

How long does the Hospital 2 Home Worker work with a patient for?

The Hospital 2 Home Service is a brief intervention service primarily supporting patients with their discharge home. Once we’ve supported someone we can provide a follow up call/ welfare check within the first few days/ week of someone being discharged from hospital.

Welcome Home - Community Support

The Welcome Home service provides support to unpaid carers and those that are cared for, living in Lambeth, who've recently been discharged from hospital.

Working with both the patient and carer for up to four weeks, providing support with, but not limited to, the following:

- Providing information and support around housing or finances,
- Signposting and making onward referrals,
- Introducing someone to a new activity or local club,
- Linking in with services and organisations in the local area,
- Helping someone to be more active.

Who is the service for?

- Lambeth residents
- Must be either over 55
- Or an unpaid carer (over 18)
- Or be over 18 with a long term health condition or disability

How long does the Welcome Home Coordinator work with a patient for?

We will work with someone for up to four weeks. If the team identifies someone needs ongoing support beyond the four weeks they will make appropriate onward referrals.

Carers Hospital Discharge Partnership

Age UK Lambeth and the Carers’ Hub Lambeth are working in partnership to offer support to unpaid carers, whilst the person they care for is in hospital and following their discharge from hospital.

As part of this partnership Age UK Lambeth is delivering:

Discharge Helpline:
This Helpline is available to unpaid carers and those that they care for, whether they’re currently in hospital, awaiting discharge or have recently been discharged.

We have a dedicated worker who is on hand to provide information and advice around what support is available to unpaid carers and those that are cared for in the hospital, support with discharge and how to access support following a stay in hospital.

The Helpline operates: Monday to Friday from 9.30am to 4.30pm (excluding bank holidays).
