Our Cancer Social Prescribing project, offers personalised, one-to-one support helping people in Lewisham affected by cancer feel more connected to their community.
Our Cancer Champions project aims to increase cancer screening and improve early detection by training volunteers to raise awareness of signs and symptoms and help share vital information within their communities.
Together, both projects aim to address health inequalities by reaching underrepresented communities, improving access to support, and ensuring people receive the information and care they need at the right time.
Cancer Social Prescribing:
• Personalised one-to-one support for people in Lewisham affected by cancer, helping them stay connected to their community and access wellbeing, social and practical services during or after treatment.
• A conversation led approach, using a Holistic Needs Assessment (HNA) to understand what matters most and tailor support to each person’s circumstances.
• Up to 8 weeks of focused help, including signposting to local activities or services and, where needed, referrals to trusted partners for specialist support such as financial advice, emotional wellbeing, transport assistance or digital inclusion.
Cancer Champions:
• Trains local volunteers to raise awareness of cancer signs and symptoms, encourage early action and promote screening within their communities.
• Builds confidence and connection by supporting volunteers to share reliable information, have everyday conversations, and take part in community or outreach events.
• Helps reduce health inequalities by equipping volunteers to reach underrepresented groups, widening access to trusted information and support across Lewisham.
Who is the Service For?
Social Prescribing:
• Age group: 18+
• Eligibility criteria: Lewisham resident, registered with a Lewisham GP, or receiving cancer treatment at Lewisham hospital
• Borough / location restrictions: Lewisham
Cancer Champions:
• Age group: Anyone aged 18+ can sign-up
How the Service Works
Cancer Social Prescribing:
Our Cancer Social Prescribing service supports people aged 18+ who live in Lewisham, are registered with a Lewisham GP, or are receiving cancer treatment at Lewisham Hospital. We help connect you with local groups, activities and practical support that can improve your wellbeing during or after cancer treatment.
You can be referred by your GP, CNS, family or friends, or you can self refer using our online form.
Once referred, a member of our team will have a conversation with you and complete a Holistic Needs Assessment (HNA) to understand what matters most to you. We then offer up to 8 weeks of personalised support, which may include linking you to social or wellbeing activities, practical services, or referring you to trusted partners for things like financial advice, emotional wellbeing support, transport assistance or digital inclusion.
Cancer Champions:
Our Cancer Champions project will train local volunteers to help raise awareness of cancer and the support and screening services available in Lewisham. Cancer Champions will have friendly, everyday conversations that help people feel informed, supported and less alone. They’ll encourage early action, signpost people to support, help reduce stigma, and share information through local networks, workplaces, social groups, events or online.
Volunteers will receive an induction, training and ongoing support so they feel confident in the role, and anyone aged 18 or over will be able to get involved. It will all begin with a relaxed, informal chat about your interests and how the role could work for you. You can register your interest using our sign up form.
