Health and social care professionals can make referrals to the Better at Home services using the information on this page.

The Better at Home services are:

• Be-a-Friend
• Befriending Plus* (see referrals guide below)
• Handyperson Service
• Hospital Discharge Support
• Online Food Shopping
• Voluntary Services Navigator* (see referrals guide below)

Please download and fill in the relevant sections of the form below and return it to bah@ageukwandsworth.org.uk.The service coordinator will then process the referral.
