Our Hospital Discharge Support service helps us to support the safe transition of hospital patients home after they are discharged.
About the Service
This service operates to assist with the transition home following a stay in hospital and prevent re-admission. Our service co-ordinator will initially meet with the recently discharged person to discuss their requirements and offer information on any additional services or care requirements. A DBS-checked and trained volunteer or Hospital Discharge staff member can then visit the individual for a short-term period.
Our volunteers are able to help with small jobs around the house, do some shopping, encourage gentle exercise, collect prescriptions and also signpost further support if required.
How do I access the service?
Friends, family, neighbours or older people who would like to refer themselves can call 020 8877 8940, email bah@ageukwandsworth.org.uk, or pop into our office at 549 Old York Road, SW18 1TQ. Office hours are Monday-Thursday 10am-3pm.
