Age UK Doncaster is seeking a compassionate and organised Dementia Pathway Co-ordinator to support the delivery of our Keep in Mind (KiM) dementia pathway.
You will act as a single point of contact for clients and carers, coordinating community-based support and ensuring timely access to services.
Key responsibilities include:
• Managing referrals and enquiries
• Case managing clients and carers along the pathway
• Coordinating interventions with partner organisations
• Carrying out planned keep-in-touch calls
• Maintaining accurate records and contributing to KPIs and case studies
You must have these essential requirements:
• Experience working in health or social care
• Experience supporting people living with dementia and/or carers
• Strong organisational and communication skills
• Confidence using IT systems and databases
• Ability to travel across Doncaster
