MYcommunity Lambeth offers short term, brief intervention, social prescribing support to individuals. We aim to reduce health and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity; by increasing people’s active involvement with their local communities, providing information and advice and connecting people into local and statutory services for practical and emotional support. The service is designed to provide person-centred referrals and assistance.
What is social prescribing?
It is a non-clinical way of helping and improving people’s lives. Social prescribing focuses on ‘what matters to me’ and takes a holistic approach to people’s health and wellbeing.
It could be introducing someone to a community group, class or supporting someone to engage with new or old hobbies. It could be helping someone access legal or financial advice or providing someone with information and advice around their housing situation. It could be supporting someone to find volunteering opportunities or helping someone get back into work.
How can I access social prescribing?
Age UK Lambeth offers two types of social prescribing support:
Working throughout Lambeth to support those aged 55 and over or those over 18 with a long-term health condition or disability.

We deliver a variety of primary care based social prescribing:
-Social Prescribing Link Workers in North Lambeth and Croxted Primary Care Networks
-Financial Link Workers in StockWellBeing and Fiveways Primary Care Networks
-Gambling Link Workers working in partnership with the Primary Care Gambling Service
-Ageing Well Link Worker working in partnership with Fiveways Primary Care and Lambeth Together
