Through this commissioned service we can provide a wide variety of support that enables people to be discharged from hospital quickly and safely back into their own home.

Trained and experienced staff can collect patients from hospital and ensure they are settled back into their home and that their surroundings are comfortable, they have food available and the heating is on. Shopping, prescriptions and pensions can be collected as part of this support and resettlement service.

Further support can also be offered through this service such as decluttering and domestic support, where a need is identified.

Staff are trained to be able to identify further needs and support, they can signpost and refer to other services provided by Age UK Lincoln and South Lincolnshire, or to those provided by statutory and voluntary organisations.

These might include Information and Advice for benefit checks and income maximisation or Befriending support to help reduce isolation and loneliness.
