Special Conditions:

Extensive local travel throughout Berkshire during work will be required, for which mileage allowance or public transport costs will be paid. 

 

Occasional evening and weekend work may be necessary, for which time off in lieu on an hour-by-hour basis is allowed.

 

The post is subject to a satisfactory Enhanced Disclosure and Barring Scheme check for Adult Workforce.

JOB SUMMARY

 

To coordinate Age UK Berkshire Service - ensuring that older people or people with a physical or sensory disability, their carers and family members are able to access Age UK Berkshire Services.    This is a hybrid role which encompasses a number of services including but not limited to:

 

Home from Hospital

Dementia Activities

Dementia Friendly Awareness

Carer’s Partnership

Keeping in Touch

Handy Person Administration

Reception/Administration

 

 

Main Duties:

 

  1. To support colleagues in the delivery of specific services during times of high demand or staff absence.
  2. To liaise with other Coordinators to ensure that a seamless service is maintained.
  3. To promote and host Dementia Friendly activities and raise awareness of dementia across Berkshire.
  4. To promote and host Carer’s Support groups including providing information and advice about Carer’s rights across Berkshire.
  5. To help support people access and understand the Health and Social Care landscape for their local area, ensuring they know their rights and entitlements.
  6. To advocate for people who are less able to do this for themselves, ensuring their wishes and choices are heard.
  7. To receive referrals and ensure response within 24 hours of notification.
  8. To carry out risk assessments for individuals and venues.
  9. To set up person centered support plans that enable individuals to live in their own homes.
  10. To enable individuals to maintain independence by assisting them to continue with everyday tasks/routine at home (shopping, prescription collection/arranging, paying bills, GP appointments and light household tasks).
  11. To provide emotional support and information and signposting of clients to other organisations, either face to face or by phone.
  12. To provide administrative support and coordinate staff home visits.
  13. To liaise with all referrers and highlight the services to health and social services and other interested parties in raising the profile of Age UK Berkshire.
  14. To provide written records of each contact, maintain confidentiality of the client and update customer database with all relevant information.
  15. To work in partnership with other voluntary and statutory services to support older people and be an ambassador for Age UK Berkshire.
  16. To ensure that the all client feedback is proactively gathered, evaluated and influences future service delivery.
  17. To undertake relevant training to maintain skills and knowledge necessary for this post.
  18. To comply with all Age UK Berkshire policies and procedures and with all relevant legislation. To comply with all provisions of the Age UK Berkshire Information Governance policy and procedures.
  19. To carry out any other duties, commensurate with the rate of pay and grading, as may from time to time be assigned by the Trustees of Age UK Berkshire.
