Home from Hospital service

Age UK Berkshire is working with Wokingham Borough Council and Bracknell Forest Council to provide a short term Home from Hospital service for Wokingham residents OVER 50 and Bracknell residents OVER 18. 

This FREE service offers practical support for you when you come home from hospital. The service is tailored to your specifc need, and can include help with things like:

  • Food shopping
  • Light household cleaning
  • Getting you to and from GP and hospital appointments
  • Ordering and collecting your medication
  • Information, support, advice and signposting to other services that can help you in the future once you have recovered

Our experienced and friendly staff will work with you for a number of weeks after you have been discharged from hospital, depending on your needs. They will check on you regularly to see how you are getting on, and provide you with support you as you recover. 

Do you live in Wokingham or Bracknell? Are you having hospital treatment soon? (This service is also available to people who are leaving hospital after an emergency admission)
