Health & Wellbeing Coordinator

Age UK Bromley & Greenwich is looking for an experienced and motivated Health & Wellbeing Coordinator to lead our Long Term Health Conditions work in Bromley.

You will coordinate and deliver practical health and wellbeing support to adults living with long-term conditions, helping them build confidence, skills, and independence. Working closely with Bromley Well and other partners, you will manage referrals, lead outreach to hard-to-reach communities, and ensure people are supported to self-manage their health through evidence-led workshops and one-to-one support. You will also line manage a small team, monitor outcomes, and help shape services so they remain person-centred and responsive to local need.

This is a varied and hands-on role suited to someone organised, empathetic, and confident working with people who have complex needs, as well as professionals across health and community services.
