Supporting you to move more, feel better, and stay independent!

Age UK Buckinghamshire have been working in partnership with Active in the Community and Leap to create a free physical activity programme for adults aged 50+ living with a long-term health condition.

What's Included?

  • If you’re eligible, you’ll receive up to 12 months of support, including:
  • A personalised 12-week physical activity plan, tailored to your needs and abilities
  • Help accessing local, affordable activity sessions
  • Ongoing encouragement to build confidence and stay motivated

Whether you’re just getting started or looking to return to exercise, we will help you find the right pace and activities for you.

Am I Eligable?

  • Residents who are 50+ years old.
  • Currently doing less than 30 minutes of activity a day.
  • Motivated to increase activity levels
  • Currently living with a long-term health condition (see approved conditions).
  • Registered at one of the 17 listed GP practices or live in one of the Opportunity Bucks wards (check your postcode by clicking here).

Exclusion Criteria:

  • Unplanned or unexpected admission to hospital because of a health condition in the last 4 weeks. If this is the case, seek advice from a healthcare professional before joining Move Together.
  • Acute coronary syndrome (a heart attack within the last 6 weeks or unstable angina)
  • Severe or poorly controlled asthma
  • Severe COPD
  • Currently accessing cardiac or cancer rehab or pulmonary services
  • Currently undergoing wider tests regarding the long term health condition(s)
  • Blood Pressure >180mmHg (systolic) or >100mmHg (diastolic)
  • Resting heart rate >100 bpm

  • Priory Surgery
  • Riverside Surgery
  • The Mandeville Practice
  • The New Surgery
  • Tower House Surgery
  • Whitehill Surgery
