Move Together is a free physical activity programme for adults aged 50+ living with a long-term health condition. It offers personalised support to help you become more active, feel better, and stay independent. Eligible participants receive a tailored activity plan, guidance to access local sessions, and ongoing encouragement for up to 12 months.
Supporting you to move more, feel better, and stay independent
Move Together Buckinghamshire is a free programme designed to help adults aged 50+ who are living with a long-term health condition become more active in a way that feels safe, achievable, and enjoyable.
Funded by Buckinghamshire Council’s Public Health Team and delivered by Active in the Community (AITC), the programme offers personalised support for up to 12 months, helping you build confidence and improve your wellbeing at your own pace.
What's Included?
If you’re eligible, you’ll receive up to 12 months of support, including:
- A personalised 12-week physical activity plan, tailored to your needs and abilities
- Help accessing local, affordable activity sessions
- Ongoing encouragement to build confidence and stay motivated
Whether you’re just getting started or looking to return to exercise, we will help you find the right pace and activities for you.
Eligibility Criteria:
- Residents who are 50+ years old.
- Currently doing less than 30 minutes of activity a day.
- Motivated to increase activity levels
- Currently living with a long-term health condition (see approved conditions).
- Registered at one of the 17 listed GP practices or live in one of the Opportunity Bucks wards (mentioned below).
