Home from Hospital in Calderdale

This service offers practical support for a few weeks after discharge from hospital. This includes help and advice at home including shopping, laundry duties, collecting prescriptions and signposting to other services if required. It gives people confidence to get back on with their lives and is a free service in Calderdale.

Home from Hospital Seamless Service in Kirklees

We are working in partnership with Community Transport in Calderdale and Kirklees to provide this service.
We offer a seamless transport and support service providing a safe supported journey home by Community Transport and met by Age UK staff or volunteer to provide immediate support at home.
You must book this service through Hospital staff in Calderdale and Kirklees: contact the coordinators on 0300 0200213 who are available Mon-Sat 10 am -5.30 pm.
Though this is separate from Home from hospital the 2 services do work side by side and compliment each other.

Dr Matt Walsh, chief officer of NHS Calderdale CCG said: “The Seamless Home from Hospital Service is another successful part of Calderdale CCG’s mission to provide effective, patient-focussed care closer to home.

“By investing in services like this, which offer support to people in their own homes, we’re making sure people get the care they need while reducing the strain on hospital services. The success of collaborative and integrated care services such as these can be seen in some of the excellent patient testimonials the service has received.”

Elizabeth Day keeps smiling with help from Age UK’s Home from Hospital Service

Elizabeth Day is 87 years old and she lives by herself at her home in Boothtown. She has lived in Halifax most of her life and has two children who both live in the south.
She has spent more than her fair share of time in hospital over the past few years.
It started with a fall when she fractured her spine and was anxious about how she would cope once she was discharged, so they suggested that she contact Age UK’s Home from Hospital Service.
She has been a regular ever since!Mrs Day and home helper Kylie
“I can’t see, I can’t hear and I can’t walk, but apart from that I’m fine!” she laughs.

Weekly Support Visits

The Home from Hospital staff visit each week and help Mrs Day with advice and information, sorting out bills but she also likes to get out of the house and do her shopping “I couldn’t go by myself.  They give me the confidence to get out. And help carry the heavy shopping!”
As well as regular support visits, they organised for Age UK to do a benefits check and for visits from one of their Active Befrienders, again to help her get out and stay active and independent.
Mrs Day has a very determined personality and has an amazingly positive outlook on life considering her health “I’m lucky” she giggles “I can see the funny side of most things. But this service has been brilliant. They have all been so helpful”.
The Home from Hospital Service is currently free and available to anyone aged 50+ who has been discharged from hospital, accident and emergency and intermediate care in Calderdale. This may be a one off visit or for a few weeks.
