💷 £ 30,181 Per Annum

Age UK Doncaster is looking for an experienced Client Journey Manager to oversee the coordination of our Keep in Mind (KiM) dementia support pathway.
You will ensure people living with dementia and their carers experience a smooth, well-managed and person-centred journey through the service.
Key responsibilities include:
• Managing referrals and waiting lists
• Coordinating the client journey across the KiM pathway
• Overseeing and quality-assuring client assessments
• Supporting and training staff to carry out assessments consistently
• Working closely with staff and partner organisations
• Monitoring activity, outcomes and KPIs
• Maintaining accurate, high-quality records
• Identifying and resolving risks or delays
You must have these essential requirements:
• Experience in health, social care or community services
• Experience carrying out, overseeing or quality-assuring assessments
• Ability to support, coach or train staff
• Experience working with partner organisations
• Excellent organisational and communication skills
• Very strong IT skills, including confident use of databases and Microsoft Office
• Ability to travel across Doncaster
• A proactive, problem-solving approach
