Our Keep in Mind service offers free social support to people living with dementia, their carers family and friends in Doncaster.

The Keep in Mind Service offers social support through an allocated Pathway Coordinator who, following the completion of a Wellbeing MOT, will offer Keep in Touch calls, as well as completing any necessary referrals whether these are internal or external.

We also offer a range of social activities and groups through our delivery partners including; Age UK Doncaster, Alzheimer's Society, Club Doncaster Foundation, Crossroads care, darts and Royal Voluntary service.

Referral Process

To make a referral to this service you should complete the referral form linked below. You can self-refer, or be referred by a family member/ carer/ friend/ professional with consent.

We will aim to contact you to confirm your referral within 48 hours (Please note this timeframe only applies to the Keep in Mind Services opening hours: Monday - Friday 8:30am - 6:00pm)

Following your referral, you will be allocated a Lead Coordinator, and booked in for a Wellbeing MOT as soon as we have availability. Then we can make any necessary referrals to partner organisations, as listed below, or external services where necessary.

Befriending or Practical Help

Free point of access for six sessions. Befriending aims to reduce social isolation & loneliness.
You will have a designated Befriender to meet with once a week for 6 weeks. A chance to have a chat, tea / coffee, share stories and help to access the local community if appropriate. We also offer practical support.

Tuesday session 09:30-15:30– Young onset Dementia (under 65’s)

Specialist sessions for people living with dementia including Maintenance Cognitive Stimulation Therapy (via referral through our Keep in Mind Service)
